1. Introduction
Welcome to Nouriva Wellness Centre Limited ("Nouriva," "we," "our," or "the Centre"). By accessing our services, booking consultations, participating in programs, or using our facilities, you agree to be bound by these Terms and Conditions.
Nouriva Wellness Centre is a licensed Class C Level 2 Clinic operating in accordance with the regulations of the Health Professions Council of Zambia (HPCZ) and applicable laws of the Republic of Zambia.
2. Scope of Services
Nouriva Wellness Centre provides holistic health, nutrition, and wellness services including but not limited to:
- Holistic Health and Wellness Coaching: Personalized guidance addressing mental, emotional, physical, and spiritual well-being to promote overall balance and vitality.
- Nutrition and Dietetics Support: Expert consultations, assessments, and customized nutrition plans tailored to individual health needs and goals.
- Management of Chronic and Genetic Conditions: Compassionate support and care for individuals living with conditions such as Sickle Cell Disease, HIV/AIDS, and Non-Communicable Diseases (NCDs).
- Support for Neurodivergent Children: Specialized programs for children with ADHD, Autism Spectrum Disorder, and anxiety, focusing on nutrition, lifestyle, and holistic wellness.
- Hormonal Health Management: Support for hormonal disorders including PCOS, endometriosis, fibroids, thyroid disease, period pains, peri-menopause, and menopause through nutrition and lifestyle interventions.
- Lifestyle and Behavior Change Coaching: Helping clients develop sustainable habits related to physical activity, stress management, sleep, and self-care.
- Community Outreach and Education: Workshops, seminars, and campaigns to raise awareness about holistic wellness, nutrition, and prevention of non-communicable diseases (NCDs).
- Research and Evidence-Based Practice: Conducting and utilizing research to continually improve services and promote best practices in holistic health and nutrition.
Important: Our services are complementary and supportive in nature and do not replace primary medical care, emergency services, or specialist treatment.
3. No Medical Emergency Services
Nouriva Wellness Centre does not provide emergency medical services. In case of a medical emergency, clients must immediately contact:
- 992 (Emergency Services Zambia)
- Or visit the nearest hospital
4. Client Responsibility
Clients agree to:
- Provide accurate and complete health information
- Disclose current medical conditions, medications, and diagnoses
- Follow recommendations responsibly
- Consult their primary healthcare provider before making significant dietary or medication changes
- Inform the Centre of any adverse reactions
Clients acknowledge that health outcomes vary and results cannot be guaranteed.
5. Informed Consent
By engaging our services, clients acknowledge that:
- Holistic Health and Nutrition-based interventions support wellness but do not guarantee cure of disease.
- Recommendations are based on professional assessment and available scientific evidence and information.
- Participation in wellness programs is voluntary.
A signed informed consent form will be required before commencement of services.
6. Fees and Payments
- All consultations, programs, and services must be paid for as agreed prior to or at the time of service.
- Fees are non-refundable once services have commenced unless otherwise agreed in writing.
- Packages must be used within the validity period stated at purchase.
- Nouriva reserves the right to revise pricing with notice.
7. Cancellation and Rescheduling Policy
- Clients must provide at least 24 hours' notice to reschedule appointments.
- Late cancellations may attract a fee.
- Missed appointments without notice will be forfeited.
8. Confidentiality and Data Protection
Nouriva Wellness Centre respects client privacy and confidentiality.
- All personal and health information is handled in accordance with applicable Zambian laws and ethical guidelines.
- Information will not be shared without written consent unless required by law.
- Client records are securely stored.
9. Intellectual Property
All materials provided by Nouriva Wellness Centre including:
- Meal plans
- Program frameworks
- Educational materials
- Branding and content
remain the intellectual property of Nouriva Wellness Centre and may not be reproduced or distributed without written permission.
10. Code of Conduct
Clients are expected to:
- Treat staff and other clients with respect
- Refrain from abusive, disruptive, or inappropriate behavior
- Comply with clinic policies
The Centre reserves the right to refuse or discontinue services where conduct compromises safety or professional standards.
11. Limitation of Liability
Nouriva Wellness Centre shall not be liable for:
- Outcomes resulting from failure to follow recommendations
- Undisclosed medical conditions
- Independent decisions made outside professional guidance
- Indirect or consequential damages
Services are provided in good faith and in accordance with professional standards.
12. Minors and Vulnerable Persons
For clients under 18 years:
- Written consent from a parent or legal guardian is required.
- A guardian may be required to attend consultations.
13. Amendments
Nouriva Wellness Centre reserves the right to update these Terms and Conditions at any time. Updated versions will be made available at the Centre or on official platforms.
14. Governing Law
These Terms and Conditions are governed by the laws of the Republic of Zambia.
